Person-centred coordinated care theory building workshop

On 25th September Debra Westlake and Dr. Helen Lloyd are running a Person-centred coordinated care (P3C) theory building workshop for researchers and practitioners from across the Southwest working on initiatives that aim to improve person centred and coordinated care for people with multiple long term conditions. They will use the data gathered to generate theories about the pre-context necessary for engaging people in meaningful involvement in these types of programme. This workshop will build on a prior theory building session with members of PenPig, Exeter and people who had been involved in the programmes locally, in which realist statements  about what works, for whom and under what circumstances were co-produced. Co-produced statements will be incorporated with the researcher and practitioner statements to develop a theoretical model which has been built from data collected from the evaluations of new care models conducted by the P3C team. The culmination of this work will form a peer reviewed publication.
